Tered C57BL 6 mice, treated with GA, compared with untreated mice and naive healthy controls. The reactivity of lymphocytes from local MLN that are adjacent to the diseased organ, as well as those from spleen cells, was analyzed. The proliferation in response to colonic extract CE ; , obtained from normal syngeneic mice, and to the treatment agent GA is depicted in Fig. 5. A prominent response to colonic extract was demonstrated by MLN cells of DSS-induced mice, i.e., 12-fold over that of naive controls. This augmented response to CE was restricted to the local MLN, because in spleens of colitis-induced mice, proliferation to CE was similar to that of naive mice. The local MLN response to CE was significantly suppressed by GA treatment, resulting in 32% inhibition. Lymphocytes from GA-treated mice from both spleens and MLN proliferated in response to GA, the systemic response being considerably higher than the response of the MLN cells. The overall secretion of two pro-inflammatory Th1 ; cytokines--TNF- and IFN -as well as of two regulatory anti

Atropine is a tertiary amine - not charged penetrates blood-brain barrier ; . Atropine methyl nitrate is charged little penetration of BBB ; . Belladonna alkaloids are rapidly absorbed from mucous membranes. Scopalamine L-Hyoscine ; Epoxidized Hyoscyamine ; Occurs in the Henbane plant Hyoscyamus niger ; and in jimson weed also called Jamestownweed ; Datura stramonium ; and a derivative, stramonium powder. The L-isomers are more potent both in peripheral and central nervous systems. Classes of Drugs Tertiary amines-no charge-penetrate CNS, e.g. atropine, scopalamine. Benztropine Cogentine ; used in treatment of Parkinsonism. Opthalmics. Quaternary analogs-charged, therefore do not tend to enter CNS. Atropine methyl nitrate. Scopalamine methylbromide. Homatropine methylbromide. Propantheline Pro-Banthine ; . Certain antihistamines, antidepressants, and antipsychotics also have cholinergic blocking properties. Mechanism Atropine does not affect acetylcholine release. However, atropine and all of the parasympatholytic drugs act as competitive antagonists at acetylcholine receptors where acetylcholine exerts a parasympathomimetic or muscarinic effect. Also, the effects of acetylcholine receptors beyond parasympathetic postganglionic endings are blocked. However, following atropine administration, acetylcholine still exerts its sympathomimetic and nicotinic effects, including acting at voluntary muscle receptors. Absorption, Distribution, Metabolism and Excretion ADME ; Variable oral absorption, normally prolonged in toxic doses due to decreased GI motility. Metabolized in liver to tropic acid, tropine and esters of tropic acid and has glucoronide conjugates formed. Elimination half-life man ; 2 - 3 hours, longer in children. Toxicity Low doses of atropine 0.044 mg kg ; decreased GI motility for 8 - 12 hours in horses. Signs Produces both peripheral and CNS symptomatology. Blocks vagus-rapid heart rate at high enough doses low doses may stimulate vagal medullary centers causing bradycardia ; . Ventricular arrhythmias and EKG changes widened QRS, prolonged QT interval ; may occur. Urinary retention, constipation. Dry mouth. Decreased gastrointestinal motility, occasionally vomiting. Colic in horses. Some mild CNS depression can occur, CNS stimulation may precede depression. Behavioral changes may occur, e.g., "disorientation", hallucinogenic in man. Mydriasis, pupils may be unreactive to light. Loss of accommodation cycloplegia ; -cannot see well up close, distant vision remains sharp. Hyperthermia, hypertension, ataxia, dysphagia. Very high doses, coma and skeletal muscle paralysis may occur. Drop of urine obtained from an atropine-poisoned animal may cause mydriasis if placed into another animal's eye. Occasionally used as a diagnostic aid. Treatment of Poisoning.

By the late 1980s these potential effects had begun to worry several interest groups. Physicians expressed concern about the infringement of their ability to select whichever drug they felt was most appropriate, while the Pharmaceutical Manufacturers Association PMA ; , now known as the Pharmaceutical Research and Manufacturers of America PhRMA ; , argued that reduced manufacturer income might affect private spending on research and develop8 ment R&D ; . Groups representing racial minorities and the poor joined the PMA in criticizing restrictive formularies for denying 9 lower--income patients access to medications. n O BRA 1990. In early 1990 Sen. David Pryor D-AR ; introduced the Pharmaceutical Access and Prudent Purchasing Act as a response to increases in prescription drug prices, which had ex10 ceeded the Consumer Price Index CPI ; by as much as 150 percent. Among other provisions, this bill would have cut costs by the compulsory substitution of "preferred" drugs for prescribed drugs within "therapeutically equivalent" categories determined by a national P&T committee. Alarmed at this prospect, a number of drug manufacturers instead offered rebates to Medicaid programs based on the "best price" given to other large purchasers, in exchange for a 11 federal mandate to eliminate restrictive state formularies. 12 This bargain was sealed 26 October 1990. Passed into law as an amendment to the several thousand-page Omnibus Budget Reconciliation Act of 1990 OBRA 1990 ; the legislation required rebates based on the greater of either a minimum percentage around 15 percent ; of the average manufacturer's price offered to all purchasers, or the difference between the Medicaid price and the manufacturer's lowest price-with the exception, after 1992, of entities such as the Department of Veterans Affairs VA ; and the Indian Health 13 Service IHS ; . In addition, rebate amounts for brand-name drugs were to increase if prices rose faster than the CPI. In return, state Medicaid programs were required to cover participating manufacturers' new medications for at least six months after Federal Drug Administration FDA ; approval. Medications that had been on the market for longer periods could be excluded from reimbursement only if they belonged to one of several "OBRAexcludable" categories benzodiazepines; cough and cold remedies; fertility drugs; smoking-cessation drugs; and cosmetic, hairgrowth, and over-the-counter [OTC] products ; , or if prior approval had not been obtained in the case of a drug for which it was required. However, OBRA 1990 mandated that prior-approval programs evaluate all requests within twenty-four hours, which made 14 them expensive and administratively cumbersome. For a number of reasons, including the swift cancellation of volH E AL T.
Plots, suggesting binding to multiple sites. The placental high-affinity [125I]RTI-55 binding site had a Kd very similar to that found for the brain, although the Bmax was somewhat lower 32 ; . In contrast, the low-affinity binding site in placenta had a lower Kd than that of brain, but its density was much greater. Drug competition studies were performed to help resolve the identity of [125I]RTI-55 binding sites; however, the results are somewhat difficult to interpret. All of the IC50 values are rather high, indicating that none of the monoamine uptake inhibitors tested was very potent in displacing all of the [125I]RTI-55 binding. Yet, the multiphasic displacement curves obtained with many of these compounds, along with the curvilinear [125I]RTI-55 Scatchard plots, indicate that the low potency values may be related, at least in part, to the fact that the drugs are displacing the ligand from several different binding sites with differing affinities. The DA uptake inhibitors benztropine and mazindol were among the most potent displacers of placental [125I]RTI-55 binding, but it is unlikely that such binding was to a DA transporter, based on previous studies of both rat 33 ; and human 15 ; placenta. The most interesting result from the drug competition studies comes from citalopram, a highly selective 5-HT uptake inhibitor. The displacement curves produced by this compound could be resolved into highand low-affinity components, and the high-affinity component yielded a Ki value consistent with the affinity of citalopram for the 5-HT transporter 31 ; . Moreover, the reported affinities of RTI-55 for the 5-HT transporter in two different cultured cell lines are essentially identical to the Kd value we obtained for the high-affinity [125I]RTI-55 binding site in rat placenta 3, 17 ; . Taken together, these results are consistent with the hypothesis that a 5-HT transporter constitutes at least part of the high-affinity binding of [125I]RTI-55 to the rat placenta. Experiments performed with the more selective ligand [3H]paroxetine provide additional evidence for the presence of a placental 5-HT transporter, thereby strengthening the case for a possible interaction of [125I]RTI-55 with this site. [3H]paroxetine bound to rat placental membranes with a Kd of pM, which is in close agreement with the reported Kd for [3H]paroxetine in purified human placental brush-border membranes 72 ; 12 ; . Furthermore, unlabeled cocaine and RTI-55 both potently displaced [3H]paroxetine binding to rat placental 5-HT transporters, which is consistent with previous results from human placenta 29 ; . On the other hand, the density of [3H]paroxetinelabeled binding sites 96 fmol mg protein ; was almost threefold less than the density of high-affinity [125I]RTI55 binding sites. Although there are several possible reasons for this difference, the results of the drug competition studies suggest that the best explanation is probably that high-affinity [125I]RTI-55 binding in the placenta includes one or more sites in addition to the 5-HT transporter. There was a very high density of low-affinity [125I]RTI55 binding sites in the rat placenta. It is interesting and bepridil.

ECG showed sinus bradycandia. The same day bupropion was discontinued. The bradycardia continued and became more pronounced the next day, when the heart rate dropped to 40 bpm and blood pressure to 90 70 Hg. An acute myocandial infarction was rubed out, and therefore an adverse drug reaction was suspected. Haboperidol was decreased to 10 mg day and bcnztnopine to 2 mg day, while the lithium dose remained the same. The bradycandia pensisted as evidenced by Holter monitor, with a minimum rate of 33 bpm and an average of 43 bpm. At this time benztropine was discontinued, as it was a possible cause of bradycandia, while haiopenidoi and lithium doses remained the same. In subsequent heart rate monitoring, a return to normal values was observed, with the heart nate stabilizing between 60 bpm and 84 bpm over the next 48 hours.

Introduction Gonadotrophin-releasing hormone agonists GnRHa ; are modifications of the parent decapeptide, GnRH, and their action is based upon reversible blockade of pituitary gonadotrophin release, resulting in suppression of ovarian function. Over the last decade, almost all in-vitro fertilization IVF ; centres have employed GnRHa for pituitary suppression before human menopausal gonadotrophin HMG ; administration. This has been shown to result in superior pregnancy rates when compared with cycles not utilizing analogues Meldrum et al., 1989; Abdalla et al., 1990; Hughes et al., 1992 ; . The age-adjusted IDR for the association of diabetes with colorectal cancer among men was 1.27 95 percent confidence interval CI ; 1.02-1.58 ; table 3 ; . After adjustment for colorectal cancer risk factors fully adjusted model ; , the IDR for the association of diabetes mellitus with colorectal cancer was 1.30 95 percent CI 1.03-1.65 ; among men. Of all colorectal cancer risk factors examined, family history of colorectal cancer was most strongly related to development of colorectal cancer IDR 1.73, 95 percent CI 1.482.03 ; . Heavy exercise at work or play was the most protective factor against development of colorectal cancer IDR 0.74, CI 0.64-0.86 ; . Other important factors associated with colorectal cancer among men included older age and smoking pipes or cigars. Factors that were significantly, but only weakly, associated with colorectal cancer incidence were a higher body mass index, eating less cereal, drinking fewer glasses of milk daily, drinking more alcohol daily, and never or seldom taking aspirin. We found a significant interaction of diabetes with current cigarette smoking, and stratification by smoking status yielded the following IDRs for the association of diabetes and colorectal cancer incidence: smokers, 0.87 95 percent CI 0.58-1.31 nonsmokers, 1.72 95 percent CI 1.282.31 ; data not shown ; . The age-adjusted IDR for the association of diabetes with colorectal cancer among women was 1.06 95 percent CI 0.84-1.34 ; table 4 ; . In the fully adjusted model, the association of diabetes with colorectal cancer was 1.16 95 percent CI 0.87-1.53 ; . As with men, family history of colorectal cancer most strongly predicted development of colorectal cancer IDR 1.71, 95 percent CI 1.48-1.98 ; . Other important predictors of colorectal cancer incidence included older age and consumption of fewer glasses of milk daily. Higher educational level and consumption of cooked vegetables daily were only weakly associated with colorectal cancer incidence. There was a significant interaction of diabetes with daily salad consumption, and stratification by salad consumption resulted in the following IDRs for the association of diabetes with colorectal cancer incidence: no daily salad, 1.48 95 percent CI 1.07-2.04 and one or more salads every day, 0.67 95 percent CI 0.38-1.19 ; data not shown ; . Restriction of the analysis to persons aged 60 years and older at baseline to eliminate persons who died at an early age from heart disease resulted in fully adjusted IDRs for the association of diabetes with colorectal cancer of 1.45 95 percent CI 1.07-1.97 ; for men and 0.99 95 percent CI 0.66-1.49 ; for women data not shown ; . SITE INVENTORY AND PATHOLOGY The aim of this unit was to introduce participants to the process of conducting a condition survey on a given heritage place. More importantly to be able to select an appropriate method for a given site and establish a diagnostic system so as to document and record problem cases and monitoring situations. The unit's resource persons were Sebastien Moriset CRATerreENSAG ; , Kofi Amekudi Ghana ; , Herman Kiriama Kenya ; , and Kassim Omar Kenya ; . This was essentially the first part of this unit and participants had an opportunity to implement the methodologies learnt to identify problems with a view to analysing them further. The participants were given a presentation by Jane Kessey on the conservation problems facing the sites of Kilwa and Kunduchi ruins in Tanzania. Methodology The delivery of this unit was through PowerPoint presentations, discussions and group exercises. The participants went into the field to try and identify problems at the two sites where they were developing management plans. Thus the presentaions were complemented with real field practical. In the field participants visited Rabai and carried out the physical and condition survey. GPS and cameras were used to record the location and distances as well as the condition of the sites. Results At the end of the unit the participants were able to identify the problems affecting the management planning sites and also document them in clear and systematic matter. This information was presented in a draft report that outlined the probable causes of these problems Participants were also able identify the boundaries of the site s ; , features within the site and the condition of the site s ; . They also drew sketch maps of the site s ; , using the tools that they had been taught in class.
