III. THE WAR AGAINST DRUGS IN THAILAND Since the drug situation in Thailand has been deteriorating, there has been a drug epidemic in many areas at both the village and community levels. The increasing of drug problems in the country tends to give more negative impacts to the economy, society and political affairs. This is partly as a result of criminal adaptation operating as a network making law enforcement officers have more difficulty in detecting criminal activities. The Government, therefore, issued many measures focusing on drug prevention and also put the drug policy in the main policy on state stability. On 14 January 2003, the Prime Minister announced a War against Drugs in Thailand. Consequently, government agencies concerning drug prevention and suppression have been assigned to determine proactive measures on drug prevention and suppression, emphasizing asset forfeiture according to the Anti - Money Laundering Act B.E.2542 1999 ; . This operation was first set for the period of 1 February - 30 April 2003 and then was extended to 10 August 2003. Before the discovery of fludarabine, chlorambucil was the traditional standard agent for several decadesand it is still administered frequently as the initial therapy in many centers. Cyclophosphamide, another alkylating agent effective against CLL, was chosen for combination therapy with fludarabine. Preclinical studies had shown that the two agents had additive or synergistic activity.8, 9 In addition, DNA interstrand crosslinks induced in CLL lymphocytes after in vitro exposure to activated cyclophosphamide, 4-hydroperoxycyclophosphamide 4-HC ; , were repaired rapidly and were not detected after 6 8 hours of incubation. However, when CLL cells were exposed to small doses of fludarabine and incubated with 4-HC, 80% of DNA crosslinks were still detectable 24 hours later.10 These results are consistent with the hypothesis that fludarabine treatment interferes with the rate of crosslink repair, giving strong support to the rationale behind combining cyclophosphamide and fludarabine. Interesting clinical results were published concerning the use of this drug combination to treat CLL and other indolent lymphoproliferative disorders.11, 12 Inhibition of DNA repair is a well understood mechanism of action common among purine analogs. In addition, the metabolism and activation of cladribine within cells to the triphosphate form are similar to the mechanism of fludarabine. Based on these observations, we evaluated the clinical efficacy of the combination of cyclophosphamide and cladribine in the treatment of patients with advanced CLL and PLL.

Acquired pure red cell aplasia PRCA ; is rare in infancy, median age of onset being usually between 50 and 60 years.1-3 The pathogenesis of the disease is immune-mediated in most cases, soluble serum inhibitors or inhibitory T cells, able to impair in vitro growth of erythroid progenitors burst-forming unit [BFU-E], and colony-forming unit [CFU-E] ; , having been demonstrated in several series of patients.1-3 PRCA is frequently associated with other autoimmune phenomena, that is, production of autoantibodies directed against erythrocytes, acetylcholinesterase, smooth muscle, and so on.2 PRCA associated with autoimmune hemolytic anemia AIHA ; has been previously reported.4 Treatment of PRCA usually employs steroids and or other immunosuppressive drugs ie, cyclosporine-A and cyclophosphamide ; , or immunomodulating agents such as immunoglobulin, with response rates to these therapies ranging from 30% to 55%.2, 3 Rituximab is a humanized, murine, monoclonal antibody MoAb ; directed against the CD20 antigen, expressed on pre-B lymphocytes and on mature B lymphocytes.5, 6 Rituximab has been demonstrated to be highly effective for in vivo B-cell depletion, B lymphocytes becoming undetectable in peripheral blood after a single infusion, and recovering only 6-9 months after discontinuation of treatment.5, 6 The antibody has been recently introduced for treatment of B-cell lymphomas.7-10 We describe the case of an 18-month-old child with immunemediated PRCA and AIHA, refractory to first- and second-line immunosuppressive therapy, who was successfully treated with anti-CD20 MoAb and clove.

These responses are mediated by neuromedin B-preferring bombesin receptors Vigne et al., 1995 ; and P2Y2 receptors Frelin et al., 1993 ; , respectively. In addition, salicylates do not induce dissociation of ET-1 ETB receptor complexes Fig. 8 ; . Thus, allosteric actions of salicylates are probably specific for ETA receptors.
