No opportunistic infection occurred in patients treated with DABdX6IL-2. Two patients with CLL developed a bacterial pneumonia and sinusitis, respectively, and two additional patients developed urinary tract infections during the study period. None of these infections were thought to be drug related, because each patient had experienced similar infections before study and all infections responded readily to antibiotics. A 46-year-old woman presented with a 1-year history of progressive left-arm numbness. A cyst below the left hemidiaphragm was discovered incidentally when a CT scan was performed to examine the thymus for a suspected tumor. A thymic mass was found. MRI indicated that the cyst contained proteinaceous fluid. The thymoma was approached through a median sternotomy and resected, but the cyst was found to be infradiaphragmatic. A separate, left paraspinal incision was made to access the retroperitoneum, and the cyst was resected. Histologic examination showed that the cyst was bronchogenic in origin. Retroperitoneal bronchogenic cysts are very rare, and only four cases have been reported in the English-language literature. CHEST 2002; 121: 13571359.

ADVERSE REACTIONS Adverse Drug Reaction Overview Adverse reactions of epinephrine include transient, moderate anxiety; feelings of overstimulation; apprehensiveness; restlessness; tremor; weakness; shakiness; dizziness; sweating; an increase in pulse rate; the sensation of a more forceful heartbeat; palpitations; pallor; nausea and vomiting; headache; and or respiratory difficulties. Ventricular arrhythmias may follow administration of epinephrine. While these symptoms occur in some patients treated with epinephrine, they are likely to be more pronounced in patients with hypertension or hypothyroidism. These signs and symptoms usually subside rapidly, especially with rest, quiet, and recumbency. Some patients may be at greater risk of developing adverse reactions after epinephrine administration. These include elderly individuals, pregnant women, and patients with diabetes. Patients with coronary artery disease are prone to more severe or persistent effects, and may experience angina. Excessive doses cause acute hypertension. Rapid rises in blood pressure have produced cerebral hemorrhage, particularly in elderly patients with cardiovascular disease. Arrhythmias, including fatal ventricular fibrillation, have been reported, particularly in patients with underlying cardiac disease or those receiving certain drugs see DRUG INTERACTIONS ; . The potential for epinephrine to produce these types of adverse reactions does not contraindicate its use in an acute life-threatening allergic reaction. DRUG INTERACTIONS Overview There are no known contraindications to the use of epinephrine in a life-threatening allergic reaction.
