Ranisetron and ondansetron are selective antagonists of 5-hydroxytryptamine type 3 5-HT, ; receptors. Granisetron is well tolerated and demonstrates substantial efficacy for the treatment of emesis in patients receiving cytotoxic drugs 1 ; . It more potent and has longer-acting antiemetic properties than ondansetron when used in connection with chemotherapy 2 ; . We have previously shown that granisetron administered IV reduces the incidence and severity of postoperative nausea and vomiting PONV ; after gynecologic surgery 3, 4 ; . However, it is more expensive than other antiemetics used for this purpose, such as droperidol and metoclopramide 5 ; . An oral granisetron preparation that is less expensive than the IV preparation but still effective in reducing emesis induced by cancer chemotherapy is now available 6 ; . We recently demonstrated that preoperative oral granisetron prevents PONV after gynecologic surgery 7 ; . However, there are no studies comparing its efficacy with another commonly used and well established oral antiemetic, domperidone, for preventing PONV. We performed this prospective, randomized.

Guidelines for loperamide use were to initiate drug at the earliest sign of a poorly formed stool or at the occurrence of 1 to more bowel movements than usual in 1 day. Loperamide was provided to patients at the initial treatment visit. Nausea or vomiting: Grade 3 despite maximal antiemetic therapy. All patients were treated with dexamethasone 10 mg i.v. and either ondansetron or granisetron before infusion of CPT-11. Ativan Wyeth-Ayerst Pharmaceuticals, Collegeville, Penn. ; or Compazine GlaxoSmithKline, Research Triangle Park, N.C. ; was also allowed at the discretion of the treating physician. Other nonhematologic toxicity: Grade 3 toxicity attributable to CPT-11 therapy. Failure to recover: Failure to recover sufficiently from toxicity to be eligible for retreatment with CPT-11 within 28 days of the start of the first cycle of CPT-11 treatment.

Crease in testosterone levels, following a midcycle gonadotropin surge. Assessment of testosterone levels in women is less straightforward than it is in men. However, some data suggest that the prevalence of androgen deficiency among women with advanced hiv disease is very high. In one study published by Dr. Grinspoon's group, 50% of hiv-infected women had decreased concentrations of free testosterone, 66% of whom had wasting syndrome Grinspoon, 1997 ; . As for measuring testosterone levels, measuring free testosterone remains the key, as shbg levels have also been shown to be elevated in women. Dr. Grinspoon also suggested that testosterone levels be assessed during the early follicular phase and with the knowledge that concomitant use of estrogen can affect the accuracy of laboratory measurements.

The growing evidence that AD and cerebrovascular disease commonly coexist and interact in the brains of older individuals is an example of how Occam's razor, or parsimony of diagnosis, may lead clinicians astray when evaluating and treating older patients.68, 69 An inclusive clinical perspective that considers both AD pathology and VaD pathology as causes for cognitive decline will become increasingly appropriate as physicians see proportionately more older patients at higher risk for multiple coexisting chronic conditions in the coming decades. To better guide the treatment of patients with mixed dementia, future studies should similarly broaden their criteria to include patients with evidence for mixed causes of dementia, rather than identifying only pure AD and VaD. Similarly, population-based studies that can provide more generalizable information on real-world patients--including better identification of the risk factors, prevalence, trajectory of cognitive decline, and survival in patients with mixed dementia-- will be important for informing clinicians, patients, and families. However, even given better information on the clinical course of patients with coexisting AD and VaD, the difficult family decisions regarding appropriate treatment goals for patients with dementia, and how these goals should change as cognitive decline progresses, will remain. Additional research into patient and family attitudes will help physicians better use medications to meet realistic treatment goals in mixed dementia and grepafloxacin.
