It is therefore an ideal case type for a clinical pathway. This study was performed at the Changi General Hospital, the first purpose-built regional hospital in Singapore, serving an estimated 800 000 people living in the eastern and northeastern sector of the country; it is an 800 bed acute care general hospital providing a comprehensive range of clinical and para-clinical services. More than 80% of the beds in the hospital are subsidized by the government, i.e. from eight bed to four bed rooms. The target population is the middle to lower income groups living in the sector. The main clinical services centre on the disciplines of general medicine, general surgery and orthopaedics. The average length of stay for patients in the hospital is between 4.8 and 5.5 days. The majority of beds are used for acute care. In July 1996, the Changi General Hospital's Medical Board decided to pilot a clinical pathway programme. The objectives of such a programme have been outlined above. A systematic approach to planning the clinical pathway programme was adopted in the hospital to ensure that a comprehensive and rational method of development evolved through rigorous research and evaluation. The pathway programme evolved through five phases in chronological order as detailed below. Phase 1 Assessment and situational analysis This phase began as the result of a strategic planning process initiated by the hospital in an effort to respond to pressures brought by outside sources such as the community and the government. The main objectives were: i ; To provide infrastructure, resources and direction for the programme. ii ; To provide a framework for future evaluation of the programme. Appointment of a steering committee In July 1996, the need for a clinical pathway programme was decided by the Medical Board and supported by top management, which included clinical chiefs and the nursing division. The idea for such a programme was first mooted by a group of hospital staff who visited some reputable hospitals in the USA to learn about best practices and to acquire knowledge on recent developments in the field of health services provision and management. This benchmarking exercise resulted in the hospital adopting two important new philosophical cornerstones the practice of evidence-based medicine and the use of clinical pathways. This was a crucial first step in introducing this new clinical process improvement tool as support from top management and clinical leaders is critical to the success of any new programme involving change within an organization. Annual screening with a fecal occult blood test FOBT ; can decrease colorectal cancer mortality by up to 33%.8 ACS guidelines indicate that a yearly FOBT is an acceptable screening method for average-risk individuals 50 years of age; combining an annual FOBT with flexible sigmoidoscopy every 5 years is preferred over the use of either test alone.3 One drawback to the most common currently used FOBTs is that they are guaiac based; they detect heme peroxidase activity and are not specific for human hemoglobin. Thus, hemoglobin from red meat, peroxidase from fruits and vegetables, and certain medications can cause false-positive reactions and need to be avoided for several days before the test. In addition, vitamin C in excess of 250 mg day ; from supplements or citrus fruits and juices may cause a false-negative guaiac test result. While these FOBTs are non-invasive and specimens can be collected at home, strict dietary and medication restrictions may decrease adherence.7, 9 Newer immunochemical assays such as InSure do not react with non-human hemoglobin or peroxidase, so food restrictions are not necessary. Immunochemical FOBTs are also more specific for lower gastrointestinal bleeding because they target the globin portion of hemoglobin, which does not survive passage through the upper gastrointestinal tract. Based on these features and published performance characteristics of immunochemical FOBTs, ACS guidelines suggest that immunochemical FOBTs such as InSure "are more patientfriendly, and are likely to be equal or better in sensitivity and specificity" relative to guaiac-based tests.3, 10 Individuals Suitable for Testing include those undergoing routine screening for colorectal lesions or other sources of bleeding in the lower gastrointestinal tract. Samples should not be collected 1 ; during, or 3 days before or after, a menstrual period; 2 ; if bleeding hemorrhoids are present; 3 ; if there is visible blood in the urine or toilet bowl; 4 ; if there are bleeding cuts on the hands; or 5 ; if the toilet contains rust or saltwater. Toilet freshener should be removed and the toilet flushed prior to sample collection. Dietary roughage may increase test sensitivity, but no dietary changes or restrictions are required. Method: This immunochemistry assay uses monoclonal, mouse antihuman hemoglobin-coated chromatography test strips with colorimetric detection. The analytical sensitivity of the assay is 50 g feces.11 It is specific for colorectal bleeding and does not detect blood from the upper gastrointestinal tract.9 Interpretive Information: Positive results indicate occult blood in the feces and should be followed up with physician consultation and possible endoscopic evaluation. | Introduction: Covariate distribution modeling CDM ; is used to generate virtual patients covariate vectors ; , based on data from an existing patient population. Covariates may be continuous weight ; or categorical sex mixed covariate types present a challenge when creating a CDM. In the standard Discrete ; method DM ; , the population is subdivided by each unique combination of categorical covariates, each with a separate multivariate normal distribution MVND ; for the continuous covariates. However, when there are many categories, some subgroups have insufficient data to build a representative model. Therefore, a novel Continuous ; method CM ; was conceived in which complete covariate vectors, including both continuous and categorical covariates, could be sampled from a single MVND. Methods: A MVND is comprised of a mean vector and a variance-covariance matrix VCVM ; . In the DM, categorical covariates are sampled from their individual distributions; continuous covariates are then generated from the proper subgroup's MVND. In the CM, all covariates are treated as continuous; the resultant single MVND is used to generate complete patient covariate vectors. Since categorical covariates e.g., X ; will have continuous values, cutoff values to assign the categorical levels are defined as the inverse of the LN cumulative distribution with mean lnX ; , sd lnX ; , and cumulative probability r. The CM and DM were applied to real and simulated data sets to compare their abilities to generate matching virtual patient distributions. Results: For the real data, both methods accurately generated the summary statistics continuous ; and proportions categorical ; of the covariates, with high precision and negligible bias. The DM results, however, were based on incomplete data. Because there were N + 1 subjects in 1 3 the subgroups where N is the number of covariates in the MVND ; , the VCVMs for these subgroups were singular, and the continuous covariates could not be generated. For the simulated data sets, the CM performed as well as the DM, except when the subgroups had markedly different continuous covariate means, for which we are aware of few clinically relevant examples. Conclusions: Compared to the DM, the CM's benefits result from analyzing the whole population instead of subsets. Including a large amount of data in the creation of the MVND enhances its stability and, as a consequence, the reliability of the generated covariate combinations. In addition, by allowing all covariates to be described by a single MVND, the number of analyses that must be performed is reduced, increasing efficiency. The CM appears to generate unbiased, precise covariates for the purposes of simulating covariate vectors in a clinical trial simulation and velcade.

There are a significant number of occurrences of real estate fraud which negatively affect the elderly, caregivers and home owners who suffer with dementia. We at the NYC Chapter have been alerted to incidences of fraud which threaten the home ownership of people with dementia and their caregivers. Elizabeth Renuart of the National Consumer Law Center wrote in Housing Policy Debate, a publication of The Fanny Mae Foundation, "An Overview of the Predatory Lending Process." She describes the frequency and incidence of Mortgage Fraud and cites examples of the elderly being approached by mortgage brokers with refinance offers that promise to provide cash, lower interest rates, and lower monthly payments and pay taxes. These refinance offers result in the opposite of any of the promises. Rather, the home owners end up with greater liability. An 81 year-old African-American home owner was approached by a mortgage broker who offered a refinance package in which the owner would retire existing unsecured debt, lower her monthly payments, cover her real estate taxes and insurance, and lower her interest rates. Once the refinance contract was in effect, none of the promises were kept. Her unsecured debt was not retired, her monthly payments were not lowered, the refinance did nothing to cover her tax and insurance, and after a two year period, her interest rate increased significantly. In another example, a 71 year-old widow received a phone call from a mortgage broker who offered to refinance her two mortgages, promising to lower her monthly mortgage payment and provide , 000.00 in cash. The cash was inviting to her since she would be able to repair her kitchen. However, once the refinance was executed, she was liable for a payment that was 80% of her total monthly income and she received no cash from the transaction.
